Hey Gery thanks for your question. It might take a forensic investigation to help determine what the actual cause was. Making sure that the installation and type of surge protective device complied with Article 285 is a place to start. The short circuit current rating must also be sufficient for the available fault current as stated in 110.10 and 285.7. You might also contact the equipment manufacturer as they are usually very interested in problems with their products.
